    • If you are a graduate students: Choose the form that starts with "MSCS".
    • If you are an undergraduate students: Choose an advising form with a name that starts with the name of your program, and also the time period covers the time that you started your program. Foe example, a BSCS student started in 2009 Spring, should use the form BSCS_ADVISNG_08F-10S. If you started your program earlier than 2008 and don't know which version to use, please consult your advisor (or contact undergraduate or graduate program director).
